Faith Wellness Collective LLC (“Faith Wellness Collective,” “we,” “us,” or “our”) respects your privacy. This policy explains what information this website collects, how we use it, and the choices you have. It applies to faithwellnesscollective.com only; care you receive as a client is governed separately, as described below.

Information we collect

Information you give us. When you submit a consultation request through this site, we collect the information you enter: your name, email address, the service you are interested in, and anything you choose to write in the message field. Providing this information is voluntary.

Information collected automatically. Our hosting provider keeps standard server logs, which may include your IP address, browser type, and the pages you visit. We do not run analytics tools, advertising trackers, or social media pixels on this site, and the site does not set cookies of its own.

Please do not include detailed clinical or medical information in the contact form. It is intended for scheduling and general questions; anything you wish to discuss in confidence belongs in a session.

How we use your information

Information submitted through this website is used solely to respond to your inquiry and, if you wish, to schedule a consultation. We do not sell your personal information, share it with third parties for their marketing, or add you to mailing lists you did not ask for.

Service providers

This website relies on a small number of service providers that process limited data on our behalf:

Netlify hosts the site and processes contact-form submissions, which are delivered to our practice email and stored in our hosting account until we delete them. Google Fonts supplies the site’s typefaces, which means your browser requests font files from Google’s servers when a page loads. The contact page embeds Google Maps to show our office location; interacting with the map is subject to Google’s own privacy policy. We have chosen not to add analytics or advertising services.

How long we keep it

Contact-form submissions are kept only as long as needed to respond to you and arrange care, and are then deleted. If you become a client, the information you provided becomes part of your intake and is handled under the clinical policies described below.

Your care is protected separately

Information you share as a client — in sessions, in clinical records, and in billing — is protected under HIPAA and applicable state law, and is governed by our Notice of Privacy Practices, which is provided to every client at intake. This website is not a patient portal, and nothing on it is intended to transmit or store treatment records.

Security

This site is served over HTTPS, and form submissions are encrypted in transit. No method of transmission or storage is perfectly secure, but we limit what we collect so that little is at stake.

Children

This website is not directed to children under 13, and we do not knowingly collect personal information from them. Inquiries about care for a child or adolescent should be made by a parent or guardian.

Your choices

You may contact us at any time to ask what information we hold from your website inquiry, to correct it, or to have it deleted. Email info@faithwellnesscollective.com and we will respond promptly.

Changes to this policy

If we change how this website handles information — for example, by adding analytics — we will update this policy and its date before doing so.
